How to submit release and Code of Conduct forms

There are two forms for participants must sign.

All competitors must sign the Liability and Media Release form, which can be downloaded and submitted on the page for editing your entries (click on your username at the top, then click on the name of the tournament under "Existing Tournament Registrations," then click on the "Entries" tab). The blank form can also be download from the sidebar. This image shows where you click.

All participants (including competitors, judges, coaches, and staff) must sign the Code of Conduct. This form can be downloaded and submitted under the "General" tab for your school, on the right side of the screen, under "School form."  (click on your username at the top, then click on the name of the tournament under "Existing Tournament Registrations," then click on the "General" tab). This image shows where you click.

We prefer that the coach or adult representative combine all of the signed Codes of Conduct into a single pdf, and upload them as one file. If that is not feasible, or if you are not affiliated with a school that is competing, please use this Google form instead.
