05. School Verification Form

The template for the school verification form is available on your tabroom registration. You should copy and paste the text to another document on your school's letterhead. After you receive a signed copy from a chool administrator, you can upload it to your tabroom registration. Specific steps are below:

1. Under your registration page for this tournament, click the "General" tab
2. "School Verification Form" should appear on the right of the screen in the "School Entry Forms" section
3. Click the down arrow to download the file
4. Copy and paste the text in the file to your school's letterhead and have a school administrator sign it
5. Upload the signed form as a PDF to the "School Verification Form" on your tabroom registration

The school verification form must be completed by Tuesday, October 8, at 5pm CT.
